Student Clinical Placement Information Form
Students must keep their information up to date in Minerva.听 The Student Clinical Placement Information Form must be updated throughout the academic year (please refer to the table below) for the planning and coordination of clinical placements. Students must enter in the Student Clinical Placement Information Form the address where they will be residing during the clinical placement, their contact information, any potential conflicts of interest and the required signed consents.
