Due to freezing rain all in-person classes and activities on Wednesday, March 11, will be cancelled. Credentialing Services

MDCM degree verifications are processed by Credentialing Services through the . There is a $60 fee per education verification request. Once the request and payment have been received, you will be able to track the status of your credentialing request by using the 鈥渧iew request status鈥� link provided in the confirmation email you will receive. Forms will be sent electronically to the requesting organization.

Please allow a four-week processing period starting from the date both the online payment and the required documents have been received.

Required Documents

  1. A signed "Authorization to Release Information" form. If the Board, Institution, or Agency does not provide such a form, please submit the 海角社区 Authorization For Release of Personal Information form.
  2. Licensing Board/Institution Credentialing Request Forms
  3. Upload Courier waybill if hard copies are required
